GET STARTED

  1. Review MOC Portfolio Program Participant Requirements
  2. Determine which option meets your needs for earning MOC Part IV credit:
    • Select a UW Health Portfolio Program Project from the list below
    • -OR-
    • If you do not see a project below that meets your needs, submit a current QI project or a NEW project idea
  3. Complete the Intake Form

COMPLETE YOUR MOC PROJECT

  1. View the UW Health Improvement Network (UWHIN) computer based trainings: Introduction to Improvement, FOCUS, and PDCA
  2. Participate in your selected QI project that has been approved for MOC Part IV credit
  3. Complete the FOCUS-PDCA (A3)
  4. Complete the Attestation/Evaluation Form

NOTE: The deadline to submit your completed UW Health MOC Portfolio Program project paperwork (Intake Form,
FOCUS-PDCA (A3), and Attestation/Evaluation Form) for 2017 is December 1, 2017.


It is the responsibility of each UW Health physician to enroll in their boards’ MOC program, including payment of the MOC enrollment fee. There is no additional fee to participate in the UW Health MOC Portfolio Program.


To enroll in your board's MOC program, go to your board's web site.


UW HEALTH MOC PROJECTS


NOTE: Though you are encouraged to work on MOC projects as a group, it is still the responsibility of each participating individual who would like to claim credit to submit his/her own Intake Form, FOCUS-PDCA (A3), and Attestation/Evaluation Form.

1. Asthma Action Plan (AAP) Improvement Project

  1. Review the Project Information Sheet (MOC/CME/CEU) for details
  2. Submit the Intake Form
    • You will need your: ABMS Member Board, Board ID Number, National Provider Identifier (NPI) Number
    • On page 2 select "I want to participate in an UW Health MOC Portfolio Program Project" In drop down menu select - Asthma Action Plan (AAP) Improvement Project
  3. View the sample AAP FOCUS-PDCA (A3) completed by Dr. Allen and her team
  4. Answer all questions in the AAP FOCUS-PDCA (A3) Project Template
    1. Documents embedded in the AAP FOCUS-PDCA (A3) Project Template include:
  5. Review historical data for your baseline
    • A link to this information will be provided in an email from the MOC email account.
  6. Submit completed AAP FOCUS-PDCA (A3) Project Template to moc@ocpd.wisc.edu
  7. Complete the Attestation/Evaluation form
    • Dr. Gail Allen will be the attestor for each submission and will receive your A3 and Attestation/Evaluation Form for review and approval.

2. Opioid Management Improvement Project

  1. Review the Project Information Sheet (MOC/CME/CEU) for details
  2. Submit the Intake Form
    • You will need your: ABMS Member Board, Board ID Number, National Provider Identifier (NPI) Number
    • On page 2 select "I want to participate in an UW Health MOC Portfolio Program Project" In drop down menu select - Opioid Management Improvement Project
  3. Download and complete the Opioid improvement (A3) Project Template , which is designed to:
    1. Facilitate the improvement by following FOCUS-PDCA using an A3
    2. Be shared with and utilized by the Clinic Manager or other team members you are working with to complete this project
  4. Submit completed Opioid improvement (A3) Project Template to moc@ocpd.wisc.edu
  5. Submit the Attestation/Evaluation form
    • You will receive a link to the Attestation/Evaluation Form once the Portfolio Program reviews your completed Opioid improvement (A3) Project Template.
    • The attestor that you identify in your intake form will receive your A3 and Attestation/Evaluation Form for review and approval.

3. Obesity Improvement Project

  1. Review the Project Information Sheet (MOC/CME/CEU) for details
  2. Submit the Intake Form
    • You will need your: ABMS Member Board, Board ID Number, National Provider Identifier (NPI) Number
    • On page 2 select "I want to participate in an UW Health MOC Portfolio Program Project" in drop down menu select - Obesity Improvement Project
    • Dr. Gail Allen will be the attestor for each submission
  3. Answer all questions in the Obesity Improvement Project FOCUS-PDCA (A3) Template
  4. Submit your completed Obesity Improvement Project FOCUS-PDCA (A3) Template to moc@ocpd.wisc.edu
  5. Complete the Attestation/Evaluation Form
    • You will receive a link to the Attestation/Evaluation Form once the Portfolio Program reviews your completed Obesity Improvement Project FOCUS-PDCA (A3) Template.
    • Dr. Gail Allen will be the attestor for each submission and will receive your A3 and Attestation/Evaluation Form for review and approval.

4. Ages and Stages Questionnaire (ASQ) Screening Improvement Project

  1. Review the Project Information Sheet (MOC/CME/CEU) for details
  2. Submit the Intake Form
    • You will need your: ABMS Member Board, Board ID Number, National Provider Identifier (NPI) Number
    • On page 2 select "I want to participate in an UW Health MOC Portfolio Program Project" in drop down menu select - Ages and Stages Questionnaire (ASQ) Screening Improvement Project
    • Dr. Gail Allen will be the attestor for each submission
  3. Answer all questions in the ASQ Screening FOCUS-PDCA (A3) Project Template
  4. Submit your completed ASQ Screening FOCUS-PDCA (A3) Project Template to moc@ocpd.wisc.edu
  5. Complete the Attestation/Evaluation Form
    • You will receive a link to the Attestation/Evaluation Form once the Portfolio Program reviews your completed ASQ Screening FOCUS-PDCA (A3) Project Template.
    • Dr. Gail Allen will be the attestor for each submission and will receive your completed A3 and Attestation/Evaluation Form for review and approval.

AFFILIATION MOC PROJECTS


NOTE: Affiliation Projects are limited to those affiliate physicians. Brief information is provided for each project.


1. Group Health Cooperative (GHC): Diabetes - A1c Goal Project

  1. Review the Project Information Sheet (MOC/CME/CEU) for details
  2. Submit the Intake Form
    • You will need your: ABMS Member Board, Board ID Number, National Provider Identifier (NPI) Number
    • For the question "UW SMPH Department/Division" > Enter GHC
    • On page 2 select "I want to participate in an UW Health MOC Portfolio Program Project" > In drop down menu select - GHC: Diabetes - A1c Goal Project
  3. View the UW Health Improvement Network (UWHIN) computer based trainings (link to instructions for access)
  4. Submit completed FOCUS-PDCA using A3 Project Template to moc@ocpd.wisc.edu with the subject: GHC-Last Name
    • Follow FOCUS-PDCA using the A3 as you participate in the project
  5. Submit the Attestation/Evaluation form
    • You will receive a link to the Attestation/Evaluation Form once the Portfolio Program reviews your completed A3.
    • Dr. Christian Kastman will be the attestor for each submission and will receive your A3 and Attestation/Evaluation Form for review.
